  • Do I need a doctor’s referral for your services?
    You do not require a doctor’s referral to be assessed and treated at CK Performance and Chiropractic. However, some extended health insurance policies will require that you submit a Doctor’s referral in order to process your claim. We recommend that you call your insurance provider to determine what they require in order to reimburse your payment.
  • How should I dress?
    We recommend that you wear loose comfortable clothing for your assessment and follow up visits so that our health practitioners can perform a thorough assessment and provide effective treatments. Try to expose the area where the problem is. For example, If it is a shoulder issue, it is best to wear a tank top, if it is a knee issue, it is best to wear shorts and if it is a back issue it is best to wear a loose top that can be easily moved for assessment purposes.   • How long will each session last?
    The time frame for your initial assessment and follow up visits will depend on the nature of your condition and the type of service you are receiving. Typically, the initial appointment will be about 1 hour long and follow ups sessions are anywhere between 20-45 minutes. It is best to come early enough that you can get changed (if needed) and ready for your appointment. This will maximize the amount of time for your one-on-one session with your doctor.
  • What to expect on your first visit?
    On your first visit to CK Performance and Chiropractic we will need you to fill out some paperwork prior to seeing the doctor, please arrive 15 minutes early for this process. Alternatively, we can email you the patient paperwork to be filled out in the comfort of your own home prior to your appointment. Once all paperwork is complete you will begin your session with the doctor which will include: A complete initial assessment discussing the following: Medical history Your current problem/concerns Current diet, supplements, medications, sleep patterns, stress levels in addition to all other pertinent lifestyle information Pain quality, what aggravates and relieves your pain Review any imaging (If applicable) Goals for treatment Next the doctor will complete a physical evaluation including: Observing and touching the area and surrounding areas Looking at the movement of the joint Testing the strength of the surrounding muscles A neurological screening will be performed to see how the nerves are functioning in the area Any other specific tests relating to your condition will be performed if needed, some of these tests may provoke your symptoms. Last, with proper consent the doctor will begin treatment which may include but is not limited to: Soft tissue techniques such as Graston or Active Release Technique Dry Needling Muscle/Tissue Flossing Cupping Adjustments Rock Taping Stretching Exercise specific to the area being treated
  • What are my payment options?
    We accept all major credit cards, HSA cards, and cash payments. Payment is due in full at the end of each session.
  • Do you take insurance?
    Our office participates with most insurance programs with Out-Of-Network benefits. To learn more about our insurance/payment options please call the office at any time (475)-269-2073. You can also find out more about your specific insurance plan by calling the member service number listed on the back of your insurance card.
  • Do you accept motor vehicle/personal injury cases?
    Yes, we accept both Motor Vehicle and Personal Injury cases at CK Performance and Chiropractic.
  • Do you take Workers Compensation cases?
    No, our office does not participate with Workers Compensation Insurance.
  • Can I make an appointment if I am not injured and don’t have any pain?
    Yes, you most certainly can! At CK Performance and Chiropractic we focus on helping you achieve the most optimal overall health and wellness. We believe that injury prevention begins with having a strong core, good alignment, full joint mobility and appropriate muscle balance related to your daily tasks and activities which we also provide and can assist with.
  • What is your cancellation and no show policy?
    Our goal is to provide quality Chiropractic care in a timely manner. In order to do so we have implemented an appointment/cancellation policy. This policy enables us to better utilize available appointments for our patients in need of our care. Cancellation Policy A patient who fails to show for their scheduled appointment, or did not notify the office within 24 hours of their scheduled appointment time, shall be subject to a "No Show" cancellation fee, which is equal to the full appointment fee scheduled that day. **In the event of an actual emergency and prior notice could not be given, consideration will be given and a one-time exception may be granted. Scheduled Appointments We understand delays can happen; however, we must try to keep our other patients and doctors on time. If you are running more then 5 minutes late, please notify the office. If a patient is 10 minutes past their appointment time, we may have to reschedule your appointment, so please let us know if you are running late. How to Cancel Your Appointment If it is necessary to cancel your scheduled appointment at CK Performance and Chiropractic, we require that you call or respond to the confirmation text message 24 hours in advance from the time of your scheduled appointment. You may call our office (475) 269-2073, during our office hours, leave a message or email Hello@ckperformancechiropractic.com within 24 hours of your appointment to cancel without being charged a cancellation fee. **If you cancel the day of your appointment without enough time for the appointment to be filled, you will be charged a same day cancellation fee. **Please note, these charges are not covered by insurance and are therefore the sole responsibility of the patient before they may reschedule an appointment.
  • What is your privacy policy?
    Privacy of personal information is an important principle to CK Performance and Chiropractic. We are committed to collecting, using and disclosing personal information responsibly and only to the extent necessary for the goods and services we provide. We have proper consent forms which will be signed by you (the patient), first before ever releasing any information to another provider or person if necessary.
